Pre-tryout Requirements

Forms to be completed online before attending tryouts:

1.    Delta Registration Form

2.    Medical History and Release Waiver

3.    Waiver and Release of Liability

The Delta Region requires that you register with them and fill out all of the above forms before attending the tryouts. 

You must also fill out the MSImpact form and bring with you.
