1.
Click on the Delta Region link.
2. Click on
Renew My Membership on the top left side of the page.
3. Scroll down. Click on "Junior Olympic Volleyball Player ".
Step 1 - Click on "Junior Player Medical Release". Fill out form once it opens. Can be filled out right on your computer. Print out with your information. Sign and bring to tryouts. (Form 1)
Step 2 - Click on "Renew my junior membership in Delta Region".
4. Verify and fill in all of the required information.
Page 1 - Verify and/or update information. Click Continue.
Page 2 - Select Club (Mississippi Impact) and complete all required fields. Click Submit.
Page 3 - Verify information. Towards bottom, select "Submit Payment with Application". You will bring this application and payment with you to try-outs. Farther down, you will see waivers. Must click all 3 "I agree . . " But first, you must click on each wavier link to open and read to be able to continue. At bottom, enter Driver's License #. Click Confirm.
Click OK.
Click OK.
5. At top of application screen, click here for Printer Friendly Version. Print out and bring to tryouts. You will only need first two pages. This is your Delta Application and Waiver and Release of Liability Form. (Form 2)
You will also get and email version of this. This may be printed out instead.
6. You will also need to fill out and bring the MS Impact Form. You can find in under the forms link above or click here. (Form 3)
7. Bring all three forms and a check for $50 to the tryouts on November 7th.
